- Copy of Birth Certificate
- Copy or proof of residency: Driver’s license and utility bill
- Sports physical dated 2010 and stamped by physician
- Current photo of each child
- Map Quest directions from your residency to Arthur Engle Park 11611 Denton Ave Hudson, Fl. 34667
Registration fee $130 for 1st child, $110 for 2nd child and a family max of $300 for 3 or more

July 12- First practice from 5:30-8:30. ALL PHYSICALS MUST BE SUBMITTED BY JULY !@ OR YOU CHILD CANNOT PRACTICE!! Practice will be Monday, Tuesday, Thursday nights 5:30-8:30, until school starts.
All paperwork must be turned in by July 19th. If players paperwork is not turned in to be certified, they cannot play in ANY games until they are certified, INCLUDING JAMBOREE (August 7th) |
